You are a motivated and experienced Social Worker who has worked extensively within a hospital setting. To join our Hospital Discharge Team, you should have a thorough understanding of the Care Act 2014, the Mental Capacity Act, and safeguarding legislation. Being a person-centred individual, you emphasise on using a strengths-based approach in your work while handling complex cases in rapidly changing environments.
What the Job Involves
As a vital part of our team, you will be completing Care Act assessments, reviews, and support planning. While promoting independence and wellbeing in service users, you will identify and implement preventative interventions. You will work hand-in-hand with a variety of health professionals, voluntary organisations, community groups, service users, and their carers to achieve positive outcomes. You will also be responsible for supporting safe and timely hospital discharges while ensuring statutory responsibilities are met.
Skills
- Qualified Social Worker with Social Work England registration
- Experience of providing care within a hospital setting
- Awareness of hospital discharge pathways
- Knowledge of the Care Act 2014, Mental Capacity Act, safeguarding legislation
- Relevant experience with strengths-based assessments and person-centred support planning
- Ability to work effectively within a multidisciplinary team
- Capability to manage competing priorities
